In brief

Healthcare in Asia varies widely within a single country. In Singapore, Hong Kong or Bangkok there are clinics of a western standard. A few hundred kilometres away the nearest facility is a district hospital with no intensive care. The problem a policy has to solve here is therefore medical evacuation within the region: transport to a centre that is able to carry out the treatment, before there is any question of transport back to the Netherlands.

That intermediate step is a separate cost item alongside repatriation and is not in every policy. Ask explicitly whether medical evacuation within the region is insured and whether the emergency assistance centre has a network in the country concerned. For factory visits, inspections or fieldwork outside the major cities this is the decisive question.

Many Asian hospitals also settle payment in advance at the desk. Always have the traveller call the emergency assistance centre first for the payment guarantee. Compare this with the situation in Europe, where public systems set the tone.

What does your premium depend on?

  • Countries within the region. Japan and Singapore have different healthcare costs from Vietnam, India or Indonesia.
  • Destination inside or outside the major cities. Fieldwork and factory visits in outlying areas increase the chance of evacuation.

  • Nature of the work on site. Inspecting a production line weighs more heavily than an office visit.
  • Local transport. Driving a hire car, scooter or motorcycle changes the risk of injury.

Where things go wrong in practice

Four points that make the difference between a policy that pays out and one that does not.

Evacuation within the region comes before repatriation

After a heart attack in an industrial area outside the city, the first move is not to the Netherlands but to the nearest hospital able to treat it, sometimes in a neighbouring country. That transport by helicopter or air ambulance is a cost item in its own right. Some policies pay only for transport back to the Netherlands and not for the intermediate step. Have this confirmed in writing before departure if you go outside the major cities.

Payment in advance at the desk

Many private clinics in the region only start treatment after a deposit or after a guarantee has been issued. A traveller who produces his own credit card runs into the spending limit and has to justify the costs afterwards. So call the emergency assistance centre first: it issues the guarantee directly to the hospital and at the same time assesses whether the proposed treatment is medically necessary.

Getting around locally

A hired scooter or motorcycle is in many Asian cities the quickest way to reach a client and at the same time the most common cause of injury. Almost all policies exclude riding without the valid licence for that vehicle, even where the hire company did not ask for it. Without a helmet or without the right licence category, cover for the injury lapses.

What you will not be paid for in this region

Not covered are: travel to areas subject to a negative travel advisory, which can change overnight after a typhoon or during political unrest; the cost of vaccinations, malaria prophylaxis and preventive medical checks before departure; and treatment of a condition that was known at the time of application and was not disclosed, as referred to in Article 7:928 of the Dutch Civil Code.

What is the difference between evacuation and repatriation?

Evacuation is transport to the nearest hospital able to handle the treatment, possibly in a neighbouring country. Repatriation is medically supervised transport back to the Netherlands. In Asia evacuation often comes first, because the patient is not stable enough for a long flight. Check whether your policy covers both and not only the journey home.

Are vaccinations and malaria tablets insured?

No. Preventive care before departure falls outside every travel insurance. That is a matter for your health insurance or for your own business costs. It does count in a claim, though: if someone catches a disease for which a recommended vaccination was available and was not given, an insurer can call the cover into question.

My employee hires a scooter; is that covered?

Only if he holds the licence required in the Netherlands for that vehicle and observes the local rules, including the requirement to wear a helmet. Hire companies in the region often do not check that, but the insurer does when there is a claim. Without the right licence category, injury to the rider is usually refused.

How quickly is help available outside the city?

That varies widely from country to country and area to area. In remote regions it can take hours before qualified transport is available. That is why it is not only the cover that matters but also the emergency assistance centre's network on the ground. Ask when applying which partners the insurer works with in the region.
