Worldwide almost never means literally everywhere. It is a description of an area to which exceptions apply: countries subject to a negative travel advisory, areas under international sanctions and usually the United States and Canada, which you insure separately. So check the destination when you book and not when you check in.
The area can also differ from section to section. The medical section may apply worldwide while liability or legal expenses are limited to Europe. Go through the sections one by one on the policy schedule. The word worldwide in a brochure says nothing about it. What distinguishes a worldwide policy is medical evacuation. At remote destinations it is not the treatment but the transport to a suitable hospital that is the biggest cost item, and the emergency assistance centre decides on that transport.

Four points that make the difference between a policy that pays out and one that does not.
Medical expenses, luggage, cancellation and liability can each have their own area. It happens that care is covered worldwide while liability stops at the border of Europe. Go through the policy schedule section by section and have any differences confirmed in writing. If the policy requires the destination to be reported and you do not do so, there is no cover for the whole trip, including for events unrelated to the country.
If your employee travels to an area which the Ministry of Foreign Affairs advises against at the time of departure, there is no cover. For countries under EU or UN sanctions an insurer is also not permitted to pay out. That sanctions clause stands apart from the rest of the conditions. If the advisory changes during the stay, check the conditions to see how long the cover continues.
At destinations without a hospital of western standard it is not the treatment itself that matters but the transport to it: an air ambulance with a doctor and equipment. The emergency assistance centre assesses whether that is medically responsible, when it happens and to which institution. If you arrange transport on your own initiative or have yourself transferred without approval, those costs are in principle at your own expense.
Excluded are loss caused by war and acts of war, active participation in disturbances and the consequences of nuclear reactions. Vaccinations, malaria prophylaxis and medical checks beforehand are preventive costs and are not paid for. High-risk activities, such as working at height, offshore or in mining, fall outside the cover unless you declared them when applying and the insurer accepted them in writing.

No. Countries subject to a negative travel advisory and areas under international sanctions fall outside the cover, and the United States and Canada are usually insured separately. Check the destination before booking and ask for written confirmation for a less common country. Confirmation by email from the insurer or from our office is sufficient as evidence.
The policy then determines whether the cover continues and for how long. Some conditions give a period in which to leave the area, others end the cover immediately. Go through that point before departure and record who within your organisation decides on returning. Also report a deteriorating situation to the emergency assistance centre straight away.
The emergency assistance centre, in consultation with the treating doctor on the spot and its own medical adviser. They weigh up whether transport is medically responsible and whether local care is adequate. That assessment is binding for the cover: if you book a flight yourself or have yourself transferred without approval, the costs are in principle at your own expense.
Yes, worldwide cover includes Europe. The question is rather whether you need that scope. If your staff travel almost solely within Europe, business travel insurance for Europe is enough and you do not pay for an area you do not use. If there are a few trips a year beyond it, one continuous worldwide cover is more practical.
