In brief

With a valid European health insurance card you are entitled in the EU, Norway, Iceland, Liechtenstein and Switzerland to necessary care within the public system, on the same terms as the residents of that country. That also means: subject to the patient contributions that apply there, and only with providers that take part in that system. If you end up in a private clinic, the card does not apply.

What neither the card nor your basic health insurance does is bring you home. Medically supervised transport to the Netherlands (by road ambulance or with a medical escort on a flight) is a considerable item within Europe too. That is the reason for travel cover over short distances.

Also note the the policy definition of Europe. That rarely coincides with the continent: some insurers include the countries around the Mediterranean, others exclude parts of Eastern Europe. If you travel further afield, choose worldwide cover.

What does your premium depend on?

  • The list of countries behind the term Europe. A wide definition including the countries around the Mediterranean costs more in premium than a strict EU list.
  • Mode of travel to the destination. Driving brings different risks from flying to a capital city.
  • Number of travel days within the area. Short distances often lead to many trips. The total number of days counts.
  • Including breakdown assistance for the vehicle. Breakdown assistance and a replacement vehicle are separate cover alongside the travel policy.
  • Winter sports and mountaineering. Recovery and rescue costs in alpine areas are only covered where that section has been ticked.
  • Number of travellers per journey. If colleagues drive to the same client together, every occupant counts.

Where things go wrong in practice

Four points that make the difference between a policy that pays out and one that does not.

The European card is not travel insurance

The card gives access to the public health system of the country you are in, subject to the patient contributions that apply there. In some countries you pay a considerable part of the bill yourself, and at a private clinic the card does not apply at all. It also covers no repatriation, no assistance and no replacement transport. Regard it as a supplement, never as a replacement.

What 'Europe' means on your policy schedule

Every insurer uses its own list of countries. One includes all the countries around the Mediterranean, including Morocco, Turkey and Tunisia; another keeps to the EU plus a handful of neighbouring countries. If you drive to a client in a country that falls just outside that list, the trip is uninsured. Check the list yourself and do not go by the word Europe.

Driving to the client

Many European work trips are made in your own car or a lease car. A travel insurance covers the occupants, not the vehicle: a breakdown on the way, recovery and a replacement vehicle fall under breakdown assistance cover with your motor insurance. Your statutory liability as a driver runs through the compulsory cover under Article 2 of the Dutch Motor Insurance Liability Act (WAM). Those are three separate policies, each with its own area of cover.

What falls outside the cover within Europe

Not covered are: patient contributions and excesses that remain at your expense under the local system but do not count as medical expenses; recovery and rescue costs for winter sports or mountaineering, unless that section has been ticked; and a breakdown of or damage to the vehicle you are travelling in. A condition that was known at the time of application and was not disclosed also falls under Article 7:928 of the Dutch Civil Code outside the cover.

Is my European health insurance card enough?

For access to the public health system it is, for the rest it is not. The card does not cover patient contributions that apply in that country, treatment in a private clinic or medical transport back to the Netherlands. It is precisely that last item that is substantial within Europe too. Take the card with you, but do not rely on it as your only provision.

Which countries fall under the Europe cover?

That differs from insurer to insurer and is set out in the conditions, not in the product name. Some policies include the countries around the Mediterranean, others limit themselves to the EU with a few neighbouring countries. Check the list before you leave, particularly for destinations in south-eastern Europe, Turkey or North Africa.

Is a breakdown with the car covered?

Not on the travel insurance. That covers the people and their luggage, not the vehicle. Recovery, repair and a replacement vehicle fall under the breakdown assistance cover with your motor or fleet insurance. Your statutory liability as a driver runs through the compulsory WAM cover. Check that travel abroad is insured on all three policies.

Why is repatriation still needed within Europe?

Because distance is not the same as simplicity. A patient who is not allowed to fly has to come back by road ambulance, with a crew and sometimes with a doctor. That is an organised transport over hundreds of kilometres. Your basic health insurance does not pay for it, whether it comes from Spain or from Thailand.
