Glacier skiing has two faces. On a marked, prepared piste on top of the glacier it is no different for your insurer from ordinary skiing: the winter sports cover applies. As soon as you ski beside the markers, you are in unmanaged glacier terrain with crevasses under snow bridges, and that is a different risk category.
Most policies follow that line. Usually excluded are skiing off the marked piste, glacier tours with rope and crampons and skiing in closed areas or areas under warning. If you want to go up onto the glacier plateau with a guide, declare it before you leave and have the acceptance confirmed in writing.

Two points that determine whether your policy pays after an accident on the glacier.
A travel or accident policy excludes as standard mountain sports off the beaten track, air sports, deep diving and speed contests. A glacier piste is prepared terrain, the snow beside it is not. A short traverse between two pistes therefore already falls outside the cover.
If off-piste skiing is included, then Article 7:952 of the Dutch Civil Code still applies: intent and recklessness are not covered. Going onto a glacier field unaccompanied during an avalanche risk, or skiing past a barrier, is treated in that way. Report an accident immediately (Article 7:941 of the Dutch Civil Code).

On a marked and prepared glacier piste usually yes, through the winter sports cover. The altitude or the glacier itself is not the problem. The exclusion starts with skiing off the markers and with tours across unmanaged glacier terrain.
That differs considerably by country and rescue service and is not fixed in advance. Reckon on a helicopter and a specialist team. Always call the emergency assistance centre first, so that they arrange the guarantee of payment before the costs mount up.
Often yes. Insurers that include glacier tours usually make it a condition that you go with a recognised guide and carry the usual equipment. Going onto the glacier field on your own initiative remains excluded even then.
Liability for injury or damage to another skier normally runs through your liability insurance, under Article 6:162 of the Dutch Civil Code. If it happens during an activity that is excluded, the insurer can refuse that cover too.
