In law a light quadricycle (brommobiel) is a moped on four wheels: registration, an AM licence and the insurance obligation all apply, even when the vehicle is standing still.

A light quadricycle has a moped registration and a design top speed of 45 kilometres per hour. That makes it a motor vehicle and brings it under the insurance obligation of Article 2 of the Dutch Motor Insurance Liability Act (WAM). That obligation attaches to the registration, not to the driving: if the registration is in your name and you do not use the vehicle for six months, third-party liability cover must still be in place or you must have the registration suspended.
To drive it you need an AM licence. Without that licence you are not an authorised driver and own-damage cover falls away after a loss; third-party liability cover does pay the other party, but the insurer may recover that amount from the driver. Four points that make the difference between a policy that pays out and one that does not.
As long as the registration is in your name, third-party liability cover must be in place on the vehicle. If you put the light quadricycle in the shed for the winter, that obligation does not stop by itself. If you do not want to pay during that period, you suspend the registration with the RDW (the Dutch vehicle authority); with a suspended registration you may not use the public road. An insurer that ends the cover reports this to the RDW and you will then receive a fine.
For a light quadricycle the AM licence is the heart of the cover. If someone drives without that licence, almost every policy treats this as an exclusion. If you lend the vehicle to a neighbour or a house guest, check that they hold an AM licence and that your policy allows lending. Some insurers work with a named regular driver and restrict cover when someone else drives on a structural basis.
Different rules apply to a light quadricycle than to a car: which parts of the road, service roads or cycle paths you may use depends on the signs in each place. If you drive on a part of the road that is prohibited for your vehicle category and something happens, that does not immediately change your cover, but it does change your position on the question of liability under Article 6:162 of the Dutch Civil Code and on how the loss is apportioned.
Outside the cover are: driving without a valid AM licence; a vehicle that has been derestricted or whose speed limiter has been altered, even where the alteration had nothing to do with the loss; wear, mechanical defects and deferred maintenance; hiring out, giving lessons or delivery work that has not been declared; driving under the influence, in which case the insurer recovers from the driver what it paid the other party; and damage caused intentionally.

Yes, for as long as the registration is in your name. The insurance obligation under Article 2 of the Dutch Motor Insurance Liability Act (WAM) attaches to the registration and not to the use. If you do not want to pay a premium during a period of non-use, you suspend the registration with the RDW. While the registration is suspended you may not use the public road, and cover for damage you cause there falls away.
Usually yes, provided the driver holds an AM licence and your policy does not name a fixed driver. Check the driver clause before you hand over the key. If someone drives without an AM licence, that is an exclusion: the other party is compensated under the WAM, but the insurer recovers that amount and your own damage goes unpaid.
No. A light quadricycle has a closed or half-closed body with seat belts, and the helmet requirement for an ordinary moped does not apply. The seat belt requirement does apply. If you do not wear a belt and are more seriously injured as a result, that may be taken into account as contributory negligence when your injury claim is settled.
You are then no longer driving the vehicle as it was type-approved and insured. Almost every policy excludes damage where the design speed has been increased, even where the speed played no part in the accident. If you buy second-hand, have the speed limiter checked to confirm it is original; as the policyholder you carry the risk of an earlier alteration.
