In brief

In the Netherlands prevention sits in three places. Part of it is organised nationally and not paid for through your health insurance, such as the population screening programmes and the national immunisation programme. Part of it is in the basic package, which is set by the government and is the same at every insurer. The rest is only in supplementary packages, with their own maximum reimbursements.

That three-way split explains most of the misunderstandings. An invitation to a population screening does not come from your insurer and costs you no excess. A preventive check-up you arrange yourself, by contrast, is rarely reimbursed from the basic insurance and often falls under a supplementary package, if that cover exists at all.

Where things go wrong in practice

Four points that determine whether you pay a preventive bill yourself.

Not all prevention runs through your insurance

The national population screening programmes and the national immunisation programme are organised and funded centrally. You receive an invitation, pay no excess and do not have to arrange anything with your insurer. That explains why you do not find them in your schedule of reimbursements. So do not draw conclusions from that about your cover.

A check-up you arrange yourself is usually supplementary or at your own expense

Preventive examinations you initiate yourself, such as a general health check, a sports medical examination or screening without a medical reason, are normally not in the basic package. Insurers sometimes offer them in a supplementary package, with a maximum reimbursement per calendar year and often only at designated providers. Without that package you pay the bill yourself.

Conditions apply within basic insurance too

Preventive care that is in the basic package has its own rules: a referral from the GP, participation in a recognised programme or an indication meeting set criteria. If those are not met, there is no reimbursement, however worthwhile the care is. The compulsory excess may also apply; GP care is exempt from it, much other care is not.

An application to extend supplementary cover can be refused

The basic health insurance is subject to duty to accept, on supplementary insurance it does not. If you want a package with prevention cover, the insurer may ask health questions, apply a waiting period or refuse you. Also work out whether the package pays for itself: supplementary cover with a low maximum reimbursement sometimes costs more in premium than the treatment you get back for it.

Is preventive care in the basic package?

In part. The basic package contains a number of preventive forms of care, subject to conditions such as a referral or a recognised programme. Other prevention is arranged nationally and runs entirely outside your insurance. What you initiate yourself, such as a general check-up, usually falls under a supplementary package or at your own expense.

Do I pay an excess for preventive care?

That depends on the type of care. GP care is exempt from the compulsory excess; much other care in the basic package is not. Nationally organised population screening costs you no excess because it is not paid for out of your insurance. For your own situation, check the schedule of reimbursements of your insurer.

Can I extend my supplementary package for prevention?

You can apply, but there is no duty to accept on supplementary insurance. The insurer may ask health questions, impose a waiting period or turn you down. Extending is usually possible around the turn of the year. Compare the extra premium with the maximum reimbursement the package offers. That ratio is not always favourable.

Does Finass Verzekert advise on this?

No. Our AFM licence covers general insurance; health insurance falls under a different category. We do not advise on it and do not act as intermediaries for it.
