In skysurfing you jump from an aircraft with a board on your feet and steer during free fall as if on a surfboard. It is a discipline within parachuting, with a higher risk of an uncontrollable spin than an ordinary jump. Insurers draw no distinction: anything called air sport falls outside cover.
On travel, personal accident and often on life insurance policies too, air sports appears among the standard exclusions, usually in wording that leaves only travelling as a passenger on a scheduled flight. In practice, cover runs through the parachute centre or the parachuting federation, with limited sums insured and only during organised activities.

Two points that weigh more heavily in air sports than in winter sports.
In addition to off-piste mountain sports, diving below a stated depth and speed contests take out policies air sports in wide terms: parachuting, hang-gliding, paragliding and anything resembling them. Skysurfing falls under this without argument, even for a one-off supervised jump.
When taking out life or disability insurance you are asked about high-risk sports. If you answer incompletely, you breach the duty of disclosure under Article 7:928 of the Dutch Civil Code and payment can be reduced or refused (Article 7:930 of the Dutch Civil Code). Jumping outside the centre’s rules may also fall under Article 7:952 of the Dutch Civil Code fall.

Yes. The health declaration asks about high-risk sports. Withholding this can cost your dependants their payment under Article 7:930 of the Dutch Civil Code. Reporting it usually leads to an additional premium, not a refusal.
As a passenger you are covered as normal up to the moment you jump. Some policies limit that to aircraft with a valid certificate of airworthiness; check that wording in advance.
